CARE CONNECTIONS (Formerly Palliative Care)
Care Connections is a free social work-driven program for residents with chronic illnesses. This program focuses on understanding goals for medical care and treatment directly from residents and connects them to the Goodwin Living resources and community supports that most closely align with those goals. Care Connections can be utilized by residents of any age and with any medical goals including curative treatment, comfort measures or anything in between.
Care Connections at Goodwin House is provided by a team of social workers that can work with residents on any level of care and even when a resident is in the hospital. These social workers work directly with residents, their chosen support system and applicable Goodwin House team members and medical providers to ensure that medical goals are discussed, documented and communicated to all those involved.
The Care Connections social work team is trained in how to discuss important issues such as disease progression and options for care based on what is most important to the patient. These discussions are encouraged early in the disease process to help guide the patient's care.
